Unless your gear setup is shit, it's mostly a training issue IMO. Know how to access your weapons and other kit efficiently and safely, and not least when it should be accessed.

If you train with the guys who coined the acronym IWFA, no it's not a gear issue (mostly). It is not well defined on the site you linked to and the examples of failed IFWA are not exactly examples Craig Douglas would use to explain it. It's an issue of getting all fucked up and tangled with another guy (hence the "in-fight" part of IFWA), then trying to pull your gun and getting off one shot before their hand on the slide causes a malfunction. Now you're trying to play keep-away with a loaded Glock and simultaneously fix the malfunction, most likely one handed. All of that could be avoided with proper IFWA, using correct timing to produce the weapon from a position where your adversary cannot easily foul your draw. IFWA is not just "how do you draw" but "when do you draw", and the principles apply to a variety of weapons to include handguns, knives, or blunt contact weapons.
