Over-reaction or Under-reaction Examples?

Do you have any examples of over-reacting or under-reacting to a situation within CQB?

At what level?

More then necessary unit strength?
Over use of less-then-lethal force?
Overuse of lethal force?
Too much focus on lesser threats?
Actions resulting in unreasonable collateral damage?
Actions resulting in unreasonable personal damage?

I have personal experience with all, and more. Bad intelligence, or too little of it is the most common cause I think.
