Dealing with doors is going to have to depend a LOT on which way the door opens, which side of the door you are on (reach across or reach near for the knob) and the layout of the room you're going into. I'd say that initial entry direction will have to depend on %'s of the room. Yes you want the hard space cleared ASAP, but you also need to weigh how likely somebody is going to be on that side of the door vs the rest of the room. If the door is in the corner, I want the first guy to take the 99% of the room instead of checking the foot gap behind the door. Sure the guy could still be behind that door in that single foot, but at that angle 2 could shoot the dude through the door without having to enter the room.
