What do you run in urban operators? How does the SMEAC or Battle Plan features change to match this setting?

SMEAC, TLP and others are just guides or templates that are not affected by the terrain. Only the content within will change.

Is there any format made for purely CQB/Urban/CT?

Service Support - Sustainment
Command and signal
What is meant by preliminaries and sustainment?
What you need to do before hand and what you need to do to reinforce the situation basically.
